To help in a year of unusually high out-patient
expenses i.e. medical care not requiring
a hospital stay, some benefit is available
towards certain out-patient expenses.
To be eligible to claim out-patient benefits
you must meet the following conditions:
Your annual
out-patient expenses must exceed the
‘annual excess’ specified
for your Plan.
Only certain medical
expenses are covered. Refer to the benefits
section to see which out-patient expenses
qualify for benefit.
To make an out-patient claim:
Collect all
your out-patient receipts for your membership
year (i.e. from one renewal date to
the next).
All receipts must have
the following information:
- Date of visit;
- Patient’s name;
- Cost of visit/session.
Calculate your total
expenses.
Deduct the annual excess.
Complete and sign the
out-patient claim form and submit along
with your receipts to: Vhi Healthcare,
IDA Business Park, Purcellsinch, Kilkenny
If you have expenses for:
Radiology
Consultants’ Fees
Approved Medical &
Surgical appliances
Pathology
Radiology
Other diagnostic tests
including MRI
you will not be able to estimate the benefit
for these expenses as the amounts payable
may vary depending on the treatment received
and the hospital/centre where it was carried
out. Simply leave this area of your claim
form blank and submit it along with your
receipts to Vhi Healthcare.
