Claims for eligible expenses
under HealthSteps Silver may be
made at the end of each insurance year.
However, if you have large expenses during
the year, you may submit up to a maximum
of one claim per quarter (based on your
next renewal date), subject to any relevant
waiting period.
To claim you need a HealthSteps Silver
claim form. This can be sent out to you
by post or you can print one from here.
Every
time you visit your doctor, dentist
or medical practitioner ask for a
receipt. All receipts should have
the following information:
- patients name
- date of visit
- cost of visit
Please ensure that X-ray receipts
state:
- number of X-rays
- type of X-rays taken
To make a claim,
simply send all your receipts with
a completed HealthSteps Silver
Claim form to us and we’ll send
you a cheque. Completed claims forms
should be sent to:
