Every time you visit your doctor, dentist or medical practitioner ask for a receipt. All receipts should have the following information:
- patients name
- date of visit
- cost of visit

Please ensure that X-ray receipts state:
- number of X-rays
- type of X-rays taken

To make a claim, simply send all your receipts with a completed HealthSteps Silver Claim form to us and we’ll send you a cheque. Completed claims forms should be sent to:
